is a viral infection of the nervous system causing pain, numbness and a rash. It is now a preventable condition with the herpes zoster vaccine. Good news–the FDA just approved the vaccine for people ages 50 to 60 in addition to ages 60 and over! I had my vaccine about 2 years ago. The vaccine is painless, and it has saved me from the concern of having this terrible condition in the future. Please consider having your vaccination soon! is most commonly seen with a radiating or shooting pain in the face, arm, leg or around the torso? In order to make a definite diagnosis a rash must be observed in the area of the shooting pain. In a recent article from the American Academy of Neurology, it has been proven that many people with a typical pain syndome of shingle (herpes zoster) will have infection as proven by spinal fluid analysis with no rash. This syndrome is called zoster sine herpete. The bottom line for the doctor evaluating various radicular style pains is that herpes zoster should always be considered and a course of antiviral medicine such as acyclovir or valcyclovir may be useful.

Recent studies from the literature show that the initiation of fluoxetine (prozac) helps promote motor recovery after acute stroke? This study, which was in the February 2011 Journal Lancet Neurology, revealed that motor recovery was significantly improved after 3 months if fluoxetine was started soon after the stroke occurred. It was felt that this promoted neuroplasticity in brain injury!

Gabapentin for treatment of carpal tunnel syndrome…

has been considered for many years as a potential option to wrist splinting and surgery. In a recent article from the European Journal of Neurology, EMG/NCV confirmed carpal tunnel syndrome was treated with either placebo or 900 mg of gabapentin per day. At the conclusion to the study there was no difference in symptom relief between the 2 groups, revealing that gabapentin was not effective for carpal tunnel symptom relief. I am very excited to be presenting a lecture at the upcoming “Back Talk” symposium on September 11th. The symposium is being sponsored by the Indiana Spine Group and will take place at the Indianapolis downtown Westin Hotel on September 10th and 11th. My talk will be regarding the neurological aspects of neck, back and extremity pain and on the differential diagnosis in order to help clinicians to sort out musculoskeletal conditions versus nerve injury conditions and syndromes. Among the topics to be included will be sciatica, pinched nerves, herniated discs, carpal tunnel syndrome, ulnar neuropathy, and polyneuropathy. Many other specialties will be presenting lectures and if you attend, I’m sure that you will learn something practical and something new that you can use in your practice. I look forward to seeing you there!

Neurological Private Practice Milestone

Recently I reached a personal milestone, which is the beginning of my 30th year in private practice and beginning my 30th year as a physician. I have been so blessed to have a wonderful wife Ryta who has given me unconditional support. My office manager Jane and I have been together for 19 years and has made my professional life so much fun and organized. Thanks! I have also had the great fortune of spending much time years ago with some neurological greats including Mark Dyken, William DeMyer, Omkar Markand, Julius Goodman, Karl Manders, John Marks, Dick French and many others along the way. I am looking forward to continuing to practice neurology for many more years to come. And no thank yous would be complete without thanking Anita Huse, Leisa Hills, Dave Mooney and Bryan Mills with IMM who have helped me in my new solo neurology practice. Dr. Larry Lett
